To expedite service of the Complaint, the court completes the necessary ser vice documents on Plaintiff's behalf. The Clerk is directed to complete a summons, a USM-285 form, three 3 Notice of Lawsuit and Request for Waiver of Service for Summons forms (AO 398), three 3 Waiver of Service of Summons forms (AO 399), o ne each for Defendants DeWitt, Bauman, and Mishner. These documents shall be addressed to Thomas Read, DPS Offender Management Administrator, 919 Ala Moana Blvd., 4th Floor Honolulu, HI 96814, who is authorized to accept a single complaint, summons, and USM 285 form for employees of the Department of Public Safety.
