July 19, 2010 Opinion or Order Filing 5 WRITTEN Opinion Signed by the Honorable James B. Zagel on 7/19/2010: Plaintiff's motion for leave to file in forma pauperis 3 is granted. The Court authorizes the Cook County Jail trust fund account officer to begin making deductions from Pla intiff's fund account in accordance with this order until the entire $350 filing fee is paid. The Clerk shall send a copy of this order to the Supervisor of Inmate Trust Fund Accounts, Cook County Dept. of Corrections Administrative Office, Division V, 2700 S. California, Chicago, IL 60608. However, Defendants Cook County, Salvador Godinez, Tom Dart, and Superintendent Moreci are dismissed as Defendants. The Clerk is directed to issue summons for Defendants Emerson and Perry, and the U nited States Marshals Service is appointed to serve them. The Clerk shall also send Plaintiff a Magistrate Judge Consent Form and Instructions for Submitting Documents along with a copy of this order.
