Filing 8 ORDER: The Court grants Plaintiff Joshua Scott's renewed application to proceed in forma pauperis #5 and waives the initial partial filing fee. Scott is warned that if he receives a lump sum in his prison trust fund account or an outside account that is sufficient to pay his outstanding filing fees, he must update his IFP application to disclose his changed finances. If he fails to do so, this case and any other pending cases will be subject to dismissal for proceeding under a false allegation of poverty. The Court authorizes and orders the trust fund officer at the Stateville Correctional Center to begin collecting monthly deductions from Scott's account for payment to the Clerk of Court toward the filing fee. The trust fund officer is also directed to ensure that proper deductions are made in all of the cases in which Plaintiff is proceeding in forma pauperis, as detailed in this order. The Clerk shall send a copy of this order to the trust fund officer at the Stateville Correctional Center. Summonses, however, shall not issue at this time. Scott is given a final chance to submit (1) a signed amended complaint that complies with this order and (2) a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If he fails to comply by November 15, 2018, the Court will summarily dismiss this case. Scott's motion for attorney representation #6 is denied without prejudice because the Court cannot determine at this early stage whether Scott is able to proceed effectively without counsel. Scott is advised that he must promptly submit a change-of address notification if he is transferred to another facility. Failure to do so may lead to dismissal of this action. Filing 4 ORDER: Plaintiff Joshua Scott has neither prepaid the filing fee nor filed an application for leave to proceed in forma pauperis. If he wants to pursue this case, he must pay the full filing fee of $400 or submit a renewed application to proceed in forma pauperis that (a) is certified by trust fund officers at all facilities where he was incarcerated for the six months prior to submission of his complaint (approximately February 27, 2018, to August 27, 2018) and (b) attaches copies of his trust fund statements showing activity in his accounts at those facilities for any portion of the six month look-back period. If Scott decides to proceed with this case and attempts to resolve his fee status, he must also show good cause in writing (in other words, provide a written explanation) why this case should not be dismissed for failure to exhaust. If Scott fails to comply with both directives by September 28, 2018, the Court will summarily dismiss this case. Scott is also advised that if he does not keep the Clerk apprised of his correct address, this case will be subject to dismissal.
