ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at the Cook County Jail to deduct $33.60 from Plaintiff's account for payment to the Clerk as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Clerk is directed to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail. Summonses, however, shall not issue as Plaintiff's complaint #1 is dismissed without prejudice for failure to state a claim. If Plaintiff wants to attempt to proceed with this lawsuit, he must submit an amended complaint that states a federal claim against proper defendants. failure to submit an amended complaint by November 28, 2018, will result in dismissal of this lawsuit for failure to state a claim. Plaintiff's motions for attorney representation #4 #7 are denied without prejudice. Plaintiff's request for a file-stamped copy of his complaint #6 is granted but Plaintiff is placed on notice that moving forward, he will not be entitled to further free copies even though he is proceeding in forma pauperis. See Lucien v. DeTella. 141 F.3d 773, 774 (7th Cir. 1998) (explaining that, even when proceeding under 28 U.S.C. 1915, litigants remain responsible for filing fees and other costs of litigation). Plaintiff must promptly submit a change-of-addressnotification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address. this action will be subject to dismissal. The Clerk is directed to send Plaintiff an amended complaint form, a blank USM-285 form. and instructions along with a copy of this order. as well as a copy of his complaint and the attachments #1 .
