Filing 9 ORDER: Plaintiff's response to the Court's previous show cause order -- "motion of cause stating why Plaintiff's case should not be dismissed" #7 -- is termed as a pending motion. The cause show order is discharged. Plaintiff is admonished that he must carefully and accurately complete all pleadings. Plaintiff's renewed application to proceed in forma pauperis #8 is denied without prejudice. Plaintiff has still failed to inform the Court of when his previous cases settled (even approximately) or the amount of the settlement (regardless of whether the funds were distributed to family members) to enable the Court to determine if he may proceed in forma or must pay the $400 filing fee in full. The case remains closed but the dismissal order #4 is amended to reflect that the matter is dismissed without prejudice for failure to comply with the Court's order requiring more information to access in forma pauperis status. If Plaintiff seeks to reinstate the case, he must timely submit a motion to reinstate along with a properly completed in forma pauperis application as previously ordered or pay the $400 filing fee. Filing 5 ORDER: By order dated September 26, 2018, the Court denied Plaintiffs application for leave to proceed in forma pauperis and ordered Plaintiff to either pay the $400 filing fee or submit an accurately completed application for leave to proceed in forma pauperis. The Court also ordered Plaintiff to show good cause in writing why the case should not be dismissed based on apparent false statements in his in forma pauperis application and to show good cause in writing why the case should not be dismissed with prejudice for his failure to fully disclose his litigation history. The Court warned Plaintiff that failure to comply with the Courts order by October 17, 2018, would result in dismissal of this case. Plaintiff has not complied with the Courts order to date and thus failed to dispel any inference that his false statements in his in forma pauperis application and his failure to fully disclose his litigation history were intentional. Accordingly, this case is dismissed with prejudice. The Court directs the Clerk of Court to terminate all pending motions as moot and enter final judgment. Civil case terminated. Having brought this action, Plaintiff remains obligated to pay the full filing fee. See 28 U.S.C. 1915(b)(1). The Clerk of Court shall collect the statutory $400 filing fee from Plaintiffs trust fund account using the mechanism in 28 U.S.C. 1915(b). The Court therefore authorizes and orders the trust fund officer at Plaintiffs place of incarceration to begin collecting monthly payments from Plaintiffs trust fund account in an amount equal to 20% of the preceding months income credited to the account. Monthly payments collected from Plaintiffs trust fund account shall be forwarded to the Clerk each time the amount in the account exceeds $10 until the full $400 filing fee has been paid. All payments shall be sent to the Clerk, United States District Court, 219 S. Dearborn St., Chicago, Illinois 60604, attn: Cashiers Desk, 20th Floor, and shall clearly identify Plaintiffs name and the case number assigned to this action. This payment obligation will follow Plaintiff wherever he may be transferred. The Clerk is directed to send a copy of this order to the trust fund officer at Plaintiffs place of incarceration.
