ORDER signed by the Honorable Edmond E. Chang. For the reasons stated in the Order, Plaintiff's application to proceed in forma pauperis #11 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $18.12 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to (1) send a copy of this order electronically (if possible) to the trust fund officer at Plaintiff's present place of incarceration, and (2) file Plaintiff's complaint #1 . Summonses, however, shall not issue at this time. The Court, on reconsideration of Plaintiff's motion for attorney representation, requests that Alison Jameen Baldwin, McDonnell, Boehnen, Hulbert & Berghoff, Ltd., 300 South Wacker Drive, 32nd Floor, Chicago, Illinois 60606, tel.: (312) 913-0001, represent Plaintiff in accordance with counsel's Trial Bar obligations under the District Courts Local Rule 83.37. Recruited counsel is within the class of users listed in the Electronic Public Access fee schedule adopted by the Judicial Conference of the United States and shall be exempt from the payment of fees for access via PACER to the electronic case files maintained in this Court for the above-captioned case. Counsel is directed to open a separate PACER account to be used only for the above-captioned case. Fees are exempt for this case only. Counsel shall not be exempt from the payment of fees incurred in connection with other uses of the PACER system in this Court. This exemption is valid for the duration of counsels participation in this matter and may be revoked at the discretion of the Court at any time. Counsel shall contact the PACER Service Center at 1-800-676 6856 or via the link below to create the new PACER account and to make any necessary arrangements for the waiver. If Plaintiff wants to proceed with this lawsuit, then he must either (1) submit a renewed application that is signed by him, certified by an appropriate official, and accompanied by a copy of trust fund statements showing activity in his accounts for the entire six-month period immediately before the suit's filing; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by 05/15/22019 will result in dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order.
