Filing 7 ORDER: By order dated August 30, 2019, the Court denied Plaintiff's application for leave to proceed in forma pauperis but granted him an opportunity to either (1) submit a renewed application that complied with the instructions in the order, or (2) pay the full statutory filing fee of $400.00. The time for Plaintiff to comply has passed, but the Court notes that he may not have received the order because, since it was issued, he was transferred to custody of the Illinois Department of Corrections. See Dkt. 6. The Court therefore will give Plaintiff until November 1, 2019, to either (1) submit a renewed application that complies with the instructions in the August 30, 2019 order, or (2) pay the full statutory filing fee of $400.00. Failure to timely comply will result in summary dismissal of this action. The Clerk of Court is directed to send Plaintiff a copy of this order along with a copy of the August 30, 2019 Order #5 . Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is signed by him, accompanied by a copy of his trust fund statements showing activity in his account or accounts for the entire six-month period immediately preceding his initiation of this action, and certified by an appropriate official; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by September 27, 2019, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the Court informed of his address maylead to dismissal of this action for failure to comply with a Court order and for want of prosecution. Plaintiff's motion for attorney representation #4 , which reflects that he has made no effort to obtain counsel on his own, is denied without prejudice to renewal should this case proceed to a pointwhere counsel becomes necessary.
