February 5, 2020 Opinion or Order Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is certified by an appropriate official and accompanied by a copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by 3-17, 2020, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the Court informed of his address may lead to dismissal of this action for failure to comply with a Court order and for want of prosecution. Plaintiff's motion for attorney representation #4 is denied as premature. See Pickett v. Chicago Transit Auth., 930 F.3d 869, 871 (7th Cir. 2019) (suggesting that motion for attorney representation is premature if filed "so early in the case that it was impossible to tell whether the plaintiff's could represent himself adequately"). The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauper is and a copy of this order. The Clerk shall also send a copy of this order to the trust fund officer at Plaintiff's present place of incarceration who is directed to assist Plaintiff in obtaining the needed certification and trust fund statement(s).
