Filing 8 ORDER: Plaintiff's renewed application for leave to proceed in forma pauperis #7 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $35.66 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to (1) send a copy of this order to the trust fund officer of the facility having custody of Plaintiff and to the Court's Fiscal Department; (2) file Plaintiff's complaint #1 ; (3) issue summonses for service of the complaint on Defendants Warden Miles, Lt. Krewer, and Sgt. Clark by the U.S. Marshal; and (4) send Plaintiff three blank USM-285 (Marshals service) forms, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must complete and return a USM-285 form for service on each Defendant. Failure to return the USM-285 forms by November 1, 2020, may result in dismissal of any unserved Defendant, as well as dismissal of this case in its entirety. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. Filing 6 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed application that is certified by a trust fund officer and that includes a copy of Plaintiff's prison trust fund statement showing activity in his account for the six months immediately preceding his filing of his complaint; or (2) pay the full statutory filing fee of $400.00. Because Plaintiff states that prison officials have denied his requests for documentation to complete the application, the Court requests that the trust fund officer at Stateville Correctional Center grant Plaintiff's reasonable requests for a certified copy of his trust fund statement. It is up to the prison to decide what policies apply when so doing, and Plaintiff must comply with those policies. Failure to comply with this order by November 1, 2020, will result in summary dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis (prisoner version) and a copy of this order. The Clerk also is directed to mail a copy of this order to the trust fund officer at Stateville Correctional Center.
