Filing 6 MOTION by Plaintiff Christopher E. Young to be allowed to proceed in the entitled cause. (ph, ) |
Filing 5 ORDER: To proceed further, Plaintiff must either (1) pay all his outstanding filing fees or (2) show good cause in writing why he should be allowed to proceed with a new lawsuit in this Court while his fees remain unpaid. Any written response to this order must include a certified copy of Plaintiff's trust fund account statement showing all transactions in his account or accounts for the entire twelve-month period preceding his response to this order. If Plaintiff was incarcerated at more than one facility during that time, he must obtain a certified trust fund account statement from each facility at which he was incarcerated. Plaintiff's failure to fully comply with this order by April 23, 2021, may result in summary dismissal of this case as well as referral of this matter to this Court's Executive Committee with a recommendation that Plaintiff's ability to initiate new actions in this Court be restricted until his filing fees have been paid in full. Filing 3 ORDER: Illinois prisoner Christopher E. Young initiated this action under 42 U.S.C. 1983, but he did not pay the Court's filing fee or apply for leave to proceed in forma pauperis. If Plaintiff wants to proceed further, he has until March 11, 2021, to either (1) prepay the $402.00 filing fee; or (2) submit an application to proceed in forma pauperis on the enclosed form along with a copy of his trust fund account statement showing all transactions to his account for the twelve-month period immediately preceding his initiation of this action (from February 2, 2020, to February 2, 2021). If Plaintiff was incarcerated at more than one facility during the look-back period, he must provide trust fund account statements from each facility at which he was incarcerated. Failure to comply with this order will result in dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order.
