ORDER: The Court grants Plaintiff's application to proceed in forma pauperis #3 and orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $15.30 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to (1) send a copy of this order to the trust fund officer of the facility having custody of Plaintiff and to the Court's Fiscal Department; (2) file Plaintiff's complaint #1 ; (3) add Sheridan Correctional Center Warden Sherwin Miles to the docket as a nominal Defendant; (4) correct the docket to identify the correctional officer Defendants as "1st Shift Unit C19 C/O Mrs. Richardson", "2nd Shift Unit C19 C/O Mr. Glidewell", and "2nd Shift Unit C19 Sergeant T.B.N."; (5) terminate Defendant Hyatt-Torres; (6) issue summonses for service of the complaint on Miles, Richardson, Glidewell, and Ojelade; and (7) send Plaintiff four blank USM-285 (Marshals service) forms, filing instructions, and a copy of this order. Plaintiff must complete and return a USM-285 form for service on each Defendant. Failure to return the USM-285 forms by 9/16/2022 may result in dismissal of any unserved Defendant, as well as dismissal of this case in its entirety. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendants.
