Applicants are required to answer every question on the application, even if the answer is zero or none. The plaintiff, however, does not provide information as to the date or dates of his last employment and his last monthly take-home pay (Question 1), nor does he list his current debt and financial obligations (Question 7). In addition, the plaintiff indicates that he has received no income in the past 12 months (Question 3) but elsewhere explains that he has been subsisting on food stamps (Question 8), calling the form's accuracy and completeness into doubt. Accordingly, by 3/27/24, the plaintiff is directed to either pay the full statutory filing fee of $405.00 or file a second application to proceed without prepaying fees that supplies all required information.
