ORDER with Statement; Plaintiff must either prepay the $402.00 filing fee or apply for leave to proceed in forma pauperis using the enclosed form and following the instructions in the Statement section below. Failure to fully comply with this order by October 29, 2021, may result in dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released.
