Filing 5 ENTRY DISCUSSING SERVICE OF PROCESS. The Plaintiff is reminded that he is responsible for serving the Defendants within 90 days from the date the Complaint was filed. Failure to do so may result in the dismissal of the action without prejudice against the unserved Defendant(s). Fed. R. Civ. P. 4 (m). Consistent with Rule 4(c)(1) of the Federal Rules of Civil Procedure, if the Plaintiff seeks the Court's assistance in serving the Defendants, he must submit for each Defendant the following: 1. A copy of the Complaint. 2. A properly completed summons in a civil action or waiver of the service of summons form (AO-399). Both forms are available at http://www.insd.uscourts.gov/forms/local-forms/civil_forms. 3. A pre-addressed envelope of adequate size and with adequate postage for mailing (the Complaint and summons or waiver forms).
