ORDER granting #2 Motion for Leave to Proceed in forma pauperis. Although he is excused from pre-paying the filing fee, he remains obligated for the full $350.00 filing fee. This summary of claims includes all the viable claims identified by the Court. All other claims are dismissed. If Mr. Crawford believes that additional claims were alleged in the complaint, but not identified by the Court, he shall have through April 6, 2021, to identify those claims. His deliberate indifference claim against Wexford shall proceed. All other claims are dismissed. The clerk is directed to change "Wexford Health of Indiana" to "Wexford of Indiana, LLC" as a defendant on the docket. The clerk is directed pursuant to Fed. R. Civ. P. 4(c)(3) to issue process to defendant Wexford of Indiana, LLC, in the manner specified by Rule 4(d).
