Filing 13 ORDER - Screening Complaint and Directing Further Proceedings; The claims against Officer D. Purtain and the Indiana Department of Correction are dismissed. The claims against Dr. Talbot, Dr. King, and Nurse L. Mahn shall proceed on the claim that they were deliberately indifferent to Mr. Bruce's serious medical needs when they instructed him to take medication prescribed for another inmate. This summary of claims includes all of the viable claims identified by the Court. All other claims have been dismissed. If the plaintiff believes that additional claims were alleged in the complaint, but not identified by the Court, he shall have through November 26, 2021, in which to identify those claims. The clerk is directed to terminate Officer D. Purtain and the Indiana Department of Correction as defendants on the docket. The clerk is directed pursuant to Fed. R. Civ. P. 4(c)(3) to issue process to defendants Wexford of Indiana, LLC; Dr. Paul Talbot, MD; Dr. King, MD; and Nurse L. Mahn in the manner specified by Rule 4(d). Process shall consist of the complaint filed, dkt #3 , applicable forms (Notice of Lawsuit and Request for Waiver of Service of Summons and Waiver of Service of Summons), and this Entry. The clerk is directed to serve Wexford of Indiana, LLC, electronically. Defendants Dr. Talbot, Dr. King, and Nurse L. Mahn are identified as employees of Wexford of Indiana, LLC. A copy of this Entry and the process documents shall also be served on Wexford electronically. Wexford is ORDERED to provide the full name and last known home address of any defendant who does not waive service if they have such information.
