Filing 11 ORDER ON MOTION TO PROCEED IN FORMA PAUPERIS - The plaintiff's motion to proceed in forma pauperis, dkt. #8 , is GRANTED to the extent that the plaintiff is assessed an initial partial filing fee of Eighteen Dollars and Thirty-Four Cents ($18.34). The plaintiff shall have through December 23, 2021, in which to pay this sum to the clerk of the district court. The plaintiff is informed that after the initial partial filing fee is paid, he will be obligated to make monthly payments of 20 percent of the preceding month's income each month that the amount in his account exceeds $10.00, until the full filing fee of $350.00 is paid. 28 U.S.C. 1915(b)(2). After the initial partial filing fee is received, a collection order will be issued to the plaintiff and the plaintiff's custodian, and the Court will screen the complaint in accordance with 28 U.S.C. 1915A. Filing 9 ORDER DIRECTING PLAINTIFF TO SUPPLEMENT MOTION FOR LEAVE TO PROCEED IN FORMA PAUPERIS - Plaintiff Eugene Lewis, an inmate currently located at Plainfield Correctional Facility, filed this civil rights action pursuant to 42 U.S.C. 1983 on October 12, 2021. Dkt. 1. Mr. Lewis' first motion for leave to proceed in forma pauperis was denied as presented, without prejudice, because the inmate trust account statement that Mr. Lewis provided was outdated. Dkt. 6. Mr. Lewis was permitted through November 15, 2021, to file an updated motion for leave to proceed in forma pauperis and attach a current copy of his trust account statement. In order for the Court to determine Mr. Lewis' eligibility for in forma pauperis status, Mr. Lewis must provide a copy of the transactions associated with his trust account for the six-month period preceding the filing of this action on October 12, 2021. Accordingly, Mr. Lewis shall have through December 15, 2021, to file his complete trust account statement to supplement his motion for leave to proceed in forma pauperis at docket #8 .
