ORDER - the Court ORDERS Mr. Lombardo to file an Amended Complaint by May 20, 2022 which addresses the deficiencies noted above. The Amended Complaint must: (a) contain a short and plain statement of the claims showing that he is entitled to relief, which is sufficient to provide the defendant with fair notice of the claim and its basis; (b) include a demand for the relief sought; (c) identify what injury he claims to have suffered and what persons are responsible for each such injury; and (d) set forth the grounds for this Court's jurisdiction. When filing his Amended Com-plaint, Mr. Lombardo may, but is not required to, utilize the form complaint attached as an exhibit to this Order. Additionally, a review of the docket in this case indicates that Mr. Lombardo has neither paid the $402 filing fee nor filed a Motion to Proceed In Forma Pauperis. Mr. Lombardo shall have through May 20, 2022 in which to either: (1) pay the $402 filing fee for this action; or (2) demonstrate that he lacks the financial ability to do so by filing a Motion to Proceed In Forma Pauperis, indicating that he is unable to prepay the fee and stating his belief that he is entitled to redress. If Mr. Lombardo files a Motion to Proceed In Forma Pauperis, he must use the form attached as an exhibit to this Order. Mr. Lombardo is advised that failure to comply with this Order may result in the dismissal of this case. (See Order). Copies to Plaintiff via U.S. Mail.
