ORDER : 1) Douglas' #2 MOTION for Leave to Proceed in forma pauperis is GRANTED. Pla to $350 filing fee as set forth below; 2) financial documentation filed by Pl indicates he lacks sufficient income to pay initial partial filing fee, and payment is therefore DEFERRED; 3) The clerk shall open an account in Douglas' name for receipt of filing fee. Clerk shall complete a Notice of Payment Form (Form EDKY 525). The clerk shall serve a copy of this Order and the Notice of Payment Form upon the Jailer/Warden of the institution in which Douglas is currently confined and upon the Office of the General Counsel for the DOC; 4) Each month Douglas' custodian shall send the clerk a pmt equal to 20% of his income for the preceding month, but only if the amount is excess of $10, until the $350 filing fee is paid; 5) Douglas must immediately advise the Clerk of any change in his address.
