Filing 6 ORDER: 1) Morton's #3 Motion for Leave to Proceed in forma pauperis is GRANTED. Section 1915(b)(1) requires a prisoner-plaintiff to pay the $350 as set forth below; 2) Within 28 days, Morton must pay $16.54 to the clerk as a partial filing fee; 3) the clerk shall open an account in Morton's name and shall complete Form EDKY 525 as set out and serve upon the jailer/warden and the office of general counsel for the DOC in Frankfort, KY; 4) each month Morton's custodian shall send payment in an amount equal to 20% of his income the preceding month, but only if that amount exceeds $10.00 and shall continue to do so until the entire $350 is paid; 5) Morton must immediately advise the Clerk of any change in his mailing address and failure to do so may result in dismissal of the case; 6) This matter stands submitted for initial screening.
