DEFICIENCY ORDER: 1) Kipi's #2 IFP is GRANTED. pla to pay the $350 as set forth below: (a) w/I 30 days Kipi shall pay $2.00 to the clerk; (b) The clerk to open an account. The clerk shall complete the Form EDKY 525 and serve a copy of this form upon the jailer/warden; (c) each month Kipi's custodian shall send the clerk payment in the amount equal to 20% of the preceding month but only if that amount exceeds $10 and shall continue until the $350 is paid in full; 2) the Clerk shall mark the assigned case number for EDKY 520 and send to Kipi; 3) Should Kipi wish to pursue his claim he must complete the complaint form describing the facts of the case; 4) Kipi must both pay the initial partial filing fee AND re-file his complaint within 30 days, or the case will be dismissed.
