Filing 5 ORDER: 1. Tippett's Motion for Leave to Proceed in forma pauperis R. #3 is DENIED WITHOUT PREJUDICE. 2. The Clerk of the Court shall mark the assigned case number on a form Civil Rights Complaint EDKY520 and send it to Tippett. The Clerk shall also send Tippett (1) AO-240 (2) EDKY Form 523. 3. Should Tippett wish to pursue this claim, he must complete and file the complaint form in its entirety, describing the facts of his case 4. In addition, Tippett must either pay the $400 filing and admin fees or (1) have the Cert of Inmate Acct certified by prison staff (2) complete the Affidavit of Assets/In Forma Pauperis Application, and (3) file both of them with the Court. 5.
