ORDER: 1) Mr. White's #6 Motion for Leave to Proceed in forma pauperis is GRANTED. prisoner-plaintiff is required to pay the S$350. filing fee in the civil action; 2) w/in 28 days pla must pay $42.54 to the clerk as a partial payment; 3) the clerk shall open an account in Mr. White's name for receipt. The clerk shall complete a Notice of Payment with (a) Mr. White's name, (b) his inmate registration number and (c) this case number. The clerk shall serve a copy of this Order and the NOP upon the Jailer/Warden where Mr. White is currently confined; 4) each month Mr. White's custodian shall send the clerk a payment in the amount equal to 20% of his income for the preceding month, but only if that amount exceeds $10.00, until the entire $350 is paid in full; 5) Mr. White must immediately advise the Clerk of any change in his current mailing address Failure to do so may result in dismissal of this case; 6) This matter stands submitted for screening.
