ORDER: 1) Adkins' #3 Motion for Leave to Proceed in forma pauperis is GRANTED; 2) the financial documentation filed by Adkins indicates he lacks funds and the payment is DEFERRED; 3) the clerk shall open an account in Adkins' name. The clerk shall complete Form EDKY 525 with (a) Adkins' name, (b) his registration number, (c) this case number and shall serve a copy upon the Jailer/Warden where Adkins is confined and upon the Office of General Counsel for the DOC in Frankfort, KY; 4) each month Adkins' custodian shall send a payment in the amount equal to 20% of his income from the preceding month, if that amount exceeds $10.00 and shall continue the payments until the entire $350 is paid; 5) Adkins must immediately advise the clerk of any change in his address.
