ORDER: 1) Adkins's #7 Motion for Leave to Proceed in forma pauperis is GRANTED. Payment of the $52 is WAIVED, however Adkins must pay the $350 filing fee; 2) Payment of the initial partial fee is WAIVED; 3) the clerk shall open an account in Adkins's name for receipt of the filing fee. The clerk shall complete a E.D. Ky. 525 and send to the Warden of the institution where Adkins is confined; 4) Adkins's custodian shall sent the clerk payment each month equal to 20% of Adkins's income the preceding month, only if that amount exceeds $10.00.
