ORDER: 1. Adams's motion for Leave to Proceed in forma pauperis is GRANTED. a. within 30 days from entry of this Order, Adams must pay $100.46 to the Clerk of this Court as an initial partial filing fee. b. The Clerk of the Court shall open an account in Adams's name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form EDKY 525 c, Each month Adams's custodian shall send the Clerk a pymt in amt equal to 20% of his income for the preceding month but only if amt in acct exceeds $10. The custodian shall continue such mthly pymts until entire $350 filing fee is paid. 2. Adams must immediately advise the Clerk's Office of any change in his current mailing address Failure to do so may result in dismissal of this case; and 3.
