December 14, 2023 Opinion or Order Filing 10 ORDER: 1) Simpson's #3 Motion for Leave to Proceed in forma pauperis IS GRANTED. the Court WAIVES the $55.00 admin fee; 2) Simpson must pay the $350.00 fee by (a) the Court will not require payment of an initial partial filing fee; (b) the Clerk shall open an account in Simpson's name. The clerk shall complete a E.D.Ky. 525 Form. The clerk shall send a copy of this order and the Notice of Payment form to the Warden, (c) Simpson's custodian shall send the clerk a payment each month from Simpson's account. The payment should be equal to 20% of Simpson's income from the previous year, if that amount exceeds $10.00; 3) Simpson must immediately advise the CLerk of a change in address.
