ORDER: 1) Johnson's complaint in 2:18-cv-152 is DISMISSED WITHOUT PREJUDICE; 2) Johnson's claims will proceed in 2:18-cv-153; 3) Johnson's motion to proceed in forma pauperis #4 is GRANTED, and Johnson's payment of $50.00 administrative fee is WAIVED. A prisoner is required to pay the $350.00 filing fee set forth below; 4) Within 30 days, Johnson must pay $15.00 as an initial filing fee. 5) Clerk shall open an account in Johnson's name and send a completed Form EDKY 525. Clerk to send a copy of this Order and the Notice of Payment Form to the warden; 6) After initial filing fee is paid, Johnson's custodian shall send a payment equal to 20% of his income. Custodian to continue payments until $350.00 is paid; 7) Clerk to send Johnson a EDKY Form 520 which Johnson is directed to complete within 30 days; 8) If Johnson fails to file amended complaint or pay the initial filing fee within 30 days, his case may be dismissed.
