ORDER: 1. Zein's current complaint #1 is DISMISSED without prejudice to his right to file a new action with this Court. 2. This specific action is STRICKEN from the Court's docket. 3. However, the Clerk's Office shall send Zein the following blank forms: a. a Civil Rights Complaint Form [E.D. Ky. 520 Form]; b. an Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; and c. a Certificate of Inmate Account Form [E.D. Ky. 523 Form]. 4. If Zein wishes to start a new civil case, he may do so by completing and filing a new action in accordance with the instructions set forth above. Signed by Judge William O.
