ORDER: 1. The Courts prior dismissal [Record No. #6 ] remains without prejudice. Once again, this allows Burke to file one or more new civil actions regarding the matters raised in his initial pleading. 2. The Clerk's Office is directed to send Burke a Sixth Circuit Form 4 and a Certificate of Inmate Account form [E.D. Ky. 523 Form]. 3. If Burke wishes to proceed with his appeal rather than file a new civil action, within 30 days from the entry of this Order, he must either pay the $505.00 appellate filing fee to the Clerk of this Court, or he must complete and file both the Sixth Circuit Form 4 and E.D. Ky. 523 Form. If he fails to take these steps within the time allotted, Burke's appeal may be dismissed. 4.
