Filing 7 ORDER: 1. If it has not already done so, the Clerk's Office is directed to update Burke's address to reflect the fact that he is now incarcerated at the Roederer Correctional Complex in LaGrange, Kentucky. 2. The Clerk's Office shall then send Burke the following blank forms: a. Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; and b. Certificate of Inmate Account Form [E.D. Ky. 523 Form]. 3. If Burke would like to proceed with this specific civil action (Case No. 2:21-cv-109-JMH), then, within 30 days from the entry of this Order, he must either (a) complete and file both the AO 240 and E.D. Ky. 523 Forms or (b) pay the $402.00 in filing and administrative fees up front and in full. If Burke fails to take these steps within the time allotted, then the Court will dismiss this specific civil action (Case No. 2:21-cv-109-JMH) without prejudice. 4. Finally, Burke must keep the Clerk's Office informed of his current mailing address. If Burke fails to notify the Clerk's Office of a change in his address, then the Court will dismiss this matter.
