Filing 7 ORDER: 1. motion #3 for appointment of counsel is DENIED; 2. motion for leave to proceed in forma pauperis is GRANTED and the 52.00 admin fee is WAIVED; 3. Stephenson must pay 350.00 filing fee as ste forth: a. Court will not require payment of an initial partial filing fee. b. Clerk open an account in Stephenson's name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form [E.D.Ky. 525 Form]. Clerk shall then send a copy ofthis Order and the Notice of Payment Form to the Warden of the institutionwhere Stephenson is currently confined.c. Stephenson's custodian shall send the Clerk a payment eachmonth from his inmate trust fund account. The payment should be in anamount equal to 20% of Stephenson's income from the preceding month. However, the custodian shall only send a monthly payment if the amount in Stephenson's account exceeds $10.00. The custodian shall continue the monthly payments until Stephensons entire $350.00 filing fee is paid. 4. Stephenson must immediately advise the Clerks Office, in writing, of any change in his mailing address. If Stephenson fails to do so, the Court may dismiss his case. 5.
