(4) All 8th Amendment medical and verbal abuse claims and all conspiracy claims against Dfts Hickey, Dr. Maria F. Marrero, Dr. Michael Growse and PA Robert D. Williams, in their individual capacities, are DISMISSED WITH PREJUDICE. Clerk directed to note on docket sheet. (5) 8th Amendment verbal abuse and medical claim against Dft Dr. Maria F. Marrer o DISMISSED WITH PREJUDICE. (7) 8th Amendment medical claims against Dft NP Quentin Moore and Dr. Maria F. Marrero shall proceed, and the Clerk's office shall prepare the documents necessary for service of process upon NP Quentin Moore and Dr. M aria F. Marrero. (8) For Dfts Moore and Marrero, Clerk shall prepare the necessary number of "Service Packets." (9) For two identified dfts, Clerk shall prepare 3 Service Packets for USM. (10) Clerk shall send Service Packets by certified m ail, docket the certified mail receipt and note in the docket the date the Service Packets were delivered to USM. (11) USM shall serve dfts. (12) Within 40 days, USM shall send a Service Report to the Clerk's office which shall be filed in to th e record. (13) Clerk directed to immediately transmit a copy of this Order to the Office of the US Atty for purpose of requiring Dr. Maria F. Marrero to respond in expedited fashion. (14) Norris shall immediately advise Clerk's office of any cha nge in mailing address. (15) Norris must communicate solely through notices or motions with the Clerk's office.
