Filing 5 MEMORANDUM OPINION & ORDER: (1) DISMISSED w/out prejudice: (a)all claims againstJudge Joseph M. Hood, AUSA Ron Walker, Adele Burt Brown, DEA Agent Jasper White, & U.S. Marshal Thomas E. Clay; & (b) all claims for damages from remaining dfts in their official capacities; (2) Clerk to prepare documents for service of process upon remaining FMC-Lex dfts, in their individual capacities: a. Jason Terris; b. Deborah A. Hickey; c. Dr. Luis A. Morales; & d. Dr. Michael Growse; (3) Clerk to prepare " ;Service Packet" consisting of following documents for service of process upon USA: a. completed summons; b. complaint; c. this order; & d. completed USM 285; (4) Clerk shall make 3 sets of copies of above-described documents, each set containin g following: a. copies of all completed summons forms issued for dfts; b. copies of all completed USM 285; c. one copy of the Complaint & all attachments 2 ; & d. one copy of this Order; (5) Clerk shall present Service Packets & copies to USM in Lex ., KY; (6) Service of Process upon dfts Jason Terris, Deborah A. Hickey, Dr. Luis A. Morales, & d. Dr. Michael Growse shall be conducted by USM in Lex., KY, by serving Service Packet <B,I>personally</b,i> upon each of them, through arrang ement w/FBOP; USM responsible for ensuring each dft successfully served w/process; (7) USM must complete service on named dfts by serving copies described in above paragraph 4 by certified or registered mail to: a. on set of copies to Civil Pr ocess Clerk at Office of the United States Attorney - EDKY; b. one set to Office of AG; c. one set to Office of Federal Bureau of Prisons in Washington, D.C.; (8) pla SHALL: a. Immediately advise Clerk's Office of any change in current mailing a ddress; Failure to do so may result in dismissal of this case; b. Communicate w/court solely through notices of motions filed w/Clerk's Office; The court will disregrad correspondence sent directly to Judge's chambers; c.
