Mesiwala, an unnamed hospital in San Bernardino, CA, A. Eddleman, Lexington Medical Center & Un iversity of Kentucky Medical Center; the claims against these named dfts are DISMISSED WITHOUT PREJUDICE; (2) dfts Dr. Ali H. Mesiwala & an unnamed hospital in San Bernardino, A are DISMISSED from this action for lack of in personam jursdiction over them; (3) summons shall issue to Deborah Hickey, Warden at FMC Lexington; (4) clerk shall prepare the docs for service; (5) clerk prepare a "Service packet" to serve upon USA; (6) clerk shall make 3 sets of copies; (7) clerk shall present t he Service Packets & copies to USM; (8) service of process upon Hickey shall be conducted by USM by serving personally on Hickey through arrangement w/BOP; (9) USM must complete service upon Hickey by certified or registered mail to USA-EDKY, Atty Ge n & BOP; (10) pla shall advise the clerk of any change in his current address; communicate w/court solely through the clerk's office - the court will disregard correspondence sent directly to the Judge's Chambers, every motion or paper must certify that he has mailed a copy to every dft/atty & state the date of mailing.
