March 14, 2011 Opinion or Order Filing 12 AMENDED MEMORANDUM OPINION & ORDER: (1) Clerk is directed to modify CM/ECF cover sheet to reflect that the Plaintiff's name is "Leslie Charles Cohen." (2) Cohen's pending 1 Motion to Proceed in forma pauperis is DENIED. (3) If Coh en intends to proceed further in this action, he must pay the $350 filing fee to the Clerk within 30 days. If he fails to pay the full fee, the Court will dismiss for failure to prosecute. (4) If case is dismissed, it will not be reinstated even if Cohen subsequently pays the filing fee. (5) If case is dismissed for want of prosecution, Cohen will not be relieved of the obligation to pay the filing fee. (6) Court may bar future civil filings by Cohen unless they are accompanied by the full filing fee. March 1, 2011 Opinion or Order Filing 7 MEMORANDUM AND ORDER: Plaintiff's request to proceed in forma pauperis pursuant to 28 U.S.C. § 1915 is granted. It is hereby ORDERED that plaintiff's claims against Eric H. Holder, Jr., Attorney General of the United Stat es; Harvey G. Lappin, Director of the Federal Bureau of Prisons; the Federal Bureau of Prisons; Metropolitan Detention Center; and the Federal Medical Center-Lexington, Kentucky are dismissed. Plaintiff's claims against individual defendants Mic hael Growse; Maria F. Marrero, Marvin Pitt, Clarissa Oldham and Jason Childress are transferred to the United States District Court for the Eastern District of Kentucky. Plaintiff shall file an amended complaint in this Court within 30 days of the da te of this Order. The case is referred to United States Magistrate Judge Lois Bloom for all pretrial proceedings.
