ORDER: 1. Williams' #5 Motion to Compel is DENIED as moot. 2. The Clerk's Office shall send Williams a copy of this Order, as well as the Court's Order at Record No. #4 . 3. The Clerk's Office shall also send Williams the following blank forms: a. Civil Rights Complaint Form [E.D. Ky. 520 Form]; b. Application to Proceed in District Court Without Prepaying Fees or Costs [AO 240 Form]; andc. Certificate of Inmate Account Form [E.D. Ky. 523 Form]. 4.
