ORDER: 1) Workmans #1 Complaint is DISMISSED w/out prejudice, and this specific action is STRICKEN from the Courts docket. 2) All pending motions are DENIED as moot; 3) However, the Clerks Office shall send Workman the following blank forms: a) Court-approved Civil Rights Complaint Form (EDKy 520); b) Application to Proceed in District Court without prepaying fees or Costs (AO 240 Form); and Certificate of Inmate Account Form (EDKy 523 Form) 4) If Workman wishes to initiate a new civil rights case, he may do so by completing and filing a new action in accordance with the instructions set forth in this Order.
