Filing 5 ORDER : (1) The Clerk's Office shall send Johnson an Application to Proceed Without Prepaying Fees or Costs [A.O. 240 Form] and a Certificate of Inmate Account Form [E.D. Ky. 523 Form]; (2) Within 30 days from the entry of this Order, Johnson must either (a) pay the $400.00 in filing and administrative fees or (b) ask to pay the filing fee in installments over time by completing and filing both the A.O. 240 and E.D. Ky. 523 Forms. If Johnson fails to take one of these steps within the time allotted, the Court will dismiss his case for failure to prosecute; (3) Johnson must also notify the Clerk's Office of any change in his address because the Court will dismiss his case if he fails to do so; and (4) Finally, since Johnson is pursuing a Federal Tort Claims Act claim against the federal government, and this type of action may be pursued either where the plaintiff resides or where the act or omission complained of occurred, Johnson may clarify whether he wants to litigate his case in this Court or in the United States District Court for the Middle District of Pennsylvania, the judicial district in which he is currently incarcerated.
