ORDER: denying without prejudice #2 Motion for Leave to Proceed in forma pauperis and motion seeking the appointment of counsel. 2. The Clerk of the Court shall mark the assigned case number on the following forms and send them to Johnson: a. a Civil Rights Complaint [EDKY Form 520]; b. an Application to Proceed Without Prepayment of Fees and Affidavit [AO Form 240]; and c. a Certificate of Inmate Account [Form EDKY 523]. 3. Johnson must complete and file the complaint form, describing the facts of his case, specifically identifying the people, dates, places, and actions which are relevant to his claims, and explain what she wants the Court to do. 4. Johnson must either pay the $400.00 filing and administrative fees or: a. have the Certificate of Inmate Account certified by prison staff; b. complete the In Forma Pauperis Application; and c. file both documents with the Court. 5.
