Mobley's claims against Director Farmer, Judge Van Meter, the Kentucky State Police, John Doe, Major Morganson, the Fayette County Detention Center, LFUCG, the LFUCG Division of Corrections, Officer Norris, the City of Lexington, the City of Nicholasville, Mayor Gorton, and Governor Beshear are DISMISSED for failure to state claim upon which relief may be granted; 3. The Deputy Clerk shall prepare one "Service Packet" for service upon Major Hall. This shall include: a completed summons form, the complaint #1 , a copy of this order, and a completed USM Form 285; 3. The USMS shall serve Defendant Hall at the Fayette County Detention Center through arrangement with that facility; 5. Mobley's #10 MOTION for Default Judgment is DENIED; and 6. Mobley must immediately advise the Clerk's Office of any change in his current mailing address. Failure to do so may result in dismissal of this Case. Signed by Judge Gregory F. Filing 9 ORDER: 1. Mobley's motion for leave to proceed in forma pauperis [R. #3 , R. #5 ] is GRANTED. Payment of the $52 administrative fee is WAIVED. Mobley must pay the $350 filing fee in the manner set forth below; 2. Based on the financial information provided by Mobley, payment of an initial partial filing fee is WAIVED; 3. The Clerk of Court shall open an account in Mobley's name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form [E.D. Ky. 525 Form] with: (a) Mobley's name; (b) his inmate registration number; and (c) this case number. The Clerk shall then send a copy of this Order and the Notice of Payment Form to the Warden of the institutionwhere Mobley is confined; 4. Mobley's custodian shall send the Clerk of this Court a payment each month from his inmate trust fund account. The payment should be in an amount equal to 20% of Mobley's income from the preceding month. However, the custodian shall only send a monthly payment if the amount in Mobleys account exceeds $10. The custodian shall continue the monthly payments until Mobley's entire $350 filing fee is paid; 5. Mobley's #6 Petition for Certiorari is DENIED; 6. Mobley's #7 MOTION for Temporary Restraining Order is DENIED; 7. This matter now stands submitted for initial screening pursuant to 28 U.S.C. 1915A and 1915(e)(2).. Signed by Judge Gregory F.
