March 1, 2022 Opinion or Order Filing 4 ORDER: 1. The Clerk of court SHALL send Glass a Petition for Writ of Habeas Corpus [EDKY Form 521], an Affidavit of Assets / In Forma Pauperis Application [Form AO-240] and a Certificate of Inmate Account [EDKY Form 523]; 2. Within 30 days from the date of this Order, Glass SHALL complete, sign, and file the habeas petition form in its entirety, providing all of the facts relevant to this claims and stating the legal grounds for the relief he seeks; 3. Within 30 days from the date of this Order, Glass SHALL also either pay the $5 filing fee or (1) have the Certificate of Inmate Account [EDKY Form 523] certified by prison staff, (2) complete the Affidavit of Assets/In Forma Pauperis Application [Form AO-240], and (3) file both of them with the Court; and 4. The Court wans Glass that it will deny the petition without prejudice if Glass does not take each of these required steps within 30 days. Signed by Judge Robert E.
