ORDER: 1. A Deputy Clerk shall prepare 2 service packet for service upon Sheila Gant and Amie Kearns. Each packet shall include: a. completed summons form, c. the complaint, c. this Order, and d. a completed USM Form 285; 2. The Deputy Clerk shall send the service packets to USMS in Lexington & note date of delivery in the docket; 3. USMS shall personally serve Gant & Kearns at the Bourbon Co. Detention Center; 4. Within 21 days after being served, dfts must answer Jacobs's complaint; 5. Jacobs must immediately advise the Clerk's Office of any change in his mailing address. ORDER: 1. Jacobs's request to proceed in forma pauperis is GRANTED and payment of $52 administrative fee is WAIVED; 2. Jacobs must pay the $350 filing fee in the manner set forth below: a. The Court will not require payment of an initial partial filing fee at this time. b. The Clerk of the Court shall open an account in Jacobs's name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form [E.D. Ky. 525 Form] with: (a) Jacobs's name, (b) his inmate registration number, and (c) this civil case number. The Clerk shall then send a copy of this Order and the Notice of Payment Form to the Warden of the institution where Jacobs is currently confined; C. Jacobs's custodian shall send the Clerk of this Court a payment each month from his inmate trust fund account. The payment should be in an amount mequal to 20% of Jacobs's income from the preceding month. However, the custodian shall only send a monthly payment if the amount in Jacobss account exceeds $10.00. The custodian shall continue the monthly payments until Jacobss entire $350.00 filing fee is paid; 3. The Clerk's office is directed to sent Jacobs a blank EDKY 520 Civil Rights Complaint Form; 4. Jacobs must immediately advise the Clerk's Office, in writing, of any change in his mailing address. If Jacobs fails to do so, the Court may dismiss his case; 5. ORDER: 1. Jacob's current motion for leave to proceed in forma pauperis #3 is DENIED w/o prejudice; 2. The Clerk's Office is directed to send Jacobs an AO 240 form and EDKY 523 form; 3. W/in 30 days from the entry of this Order, Jacobs must either pay the $402 filing/admin fee, or properly asking to pay the filing fee in installments over time by completing and filing the AO 240 and EDKY 523 forms; 4.
