Filing 7 ORDER: 1. The Clerk's Office is direct to send Price a Pro Se 7 Complaint for Employment Discrimination Form; 2. Within 30 days from the entry date of this Order, Price Price must complete the Pro Se 7 Form and file it with this Court. In completing that form, Price must state how each defendant was involved and what each defendant did that caused her harm or violated her rights, including the dates and places of that involvement or conduct. If more than one claim is asserted, Price should number each claim and write a short and plain statement of each claim in a separate paragraph. Price may attach additional pages if needed. It is important for Price to complete the Pro Se 7 Form in its entirety because that completed form will replace her original pleading [R. 1] for all purposes; 3. If Price fails to complete & file Pro Se 7 Form within the time allotted, the Court will dismiss her case; 4. Finally, Price must keep the Clerk's Office informed of her current mailing address.
