September 28, 2018 Opinion or Order Filing 13 ORDER: 1) Lighthourn's motion for leave to proceed in forma pauperis [R. 11] is DENIED WITHOUT PREJUDICE; 2) The Clerk of the Court SHALL send Lightbourn a Sixth Circuit Form 4 and a Certificate of Inmate Account form [E.D. Ky 523 form]; 3) Within 30 days from the entry of this Order, Lighthourn must either pay t he $505.00 appellate filing fee to the Clerk of this Court or complete the following steps in order to properly pursue pauper status: a. complete the Sixth Circuit Form 4; b. have prison staff complete and sign the Certificate of Inmate Account form; and c. file both documents with this Court. If Lightbourn fails to take these steps within the time allowed, his appeal may be dismissed; 4) The Clerk of the Court SHALL forward a copy of this Order to the Clerk of the U.S. Court of Appeals for the Sixth Circuit.
