ORDER: The Court ORDERS as follows: 1. The court DENIES Surles's IFP motion DE #6 , without prejudice; 2. Clerk's office shall send Surles another Application to Proceed in District Court Without Prepaying Fees or Costs and Certificate of Inmate Account Form; and 3. Within 30 days from entry of this Order, Surles shall either (a) pay $400.00 in filing and administrative fees or (b) properly move for leave to proceed IFP by completing and filing A.O. 240 and E.D. Ky. 523 forms, including having a prison official complete and sign the bottom half of the E.D. Ky. 523 Form. Failure to take these steps within time allotted, the Court will dismiss case for failure to prosecute, without allowing further curative opportunity. Signed by Judge Robert E. Wier on 08/01/2019. Ky. 523 Forms, including having a prison official complete and sign the bottom half of the E.D. Ky. 523 Form.
