Sanders's motion for leave to appeal in forma pauperis R. #11 is DENIED, WITHOUT PREJUDICE; 2. The Clerk of the Court shall send Sanders a Sixth Circuit Form 4 and a Certificate of Inmate Account [EDKY Form 523]; 3. Within thirty (30) days, Matthews must either pay the $505.00 appellate filing fee to the Clerk of the Court or comply with the following steps to pursue pauper status: (a) complete the Sixth Circuit Form 4; (b) have prison staff complete and sign the Certificate of Inmate Account; and (c) file both documents with the Court.; 3.
