Filing 7 ORDER : 1. The Deputy Clerk shall prepare a Service Packet for service upon the United States of America, the Office of the Attorney General, and Federal Correctional Officer Jordan Perry. Each Service Packet shall include: a-d. 2. Clerk deliver packets to USMS, Lexington. 3. USMS service USA by certified or registered mail. 4. USMS personally service Jordan Perry. 5. Filing 6 ORDER: 1. Daviss motion to proceed in forma pauperis [R. #2 ] is GRANTED. 2. Within twenty-eight (28) days, Davis must pay $9.85 to the Clerk of the Court as an initial partial filing fee. 3. The Clerk shall open an account in Daviss name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form (Form EDKY 525) containing: (1) Davis's name, (2) his inmate registration number, and (3) this case number. The Clerk shall serve copy of this Order and the Notice of Payment Form upon the warden of the institution in which Davis is currently confined; 4. Each month Davis's custodian shall send the Clerk of the Court a payment in an amount equal to twenty percent (20%) of Daviss income for the preceding month out of his inmate trust fund account, but only if the amount in the account exceeds $10.00. The custodian shall continue such monthly payments until the entire $350.00 filing fee is paid. 5. Davis must immediately advise the Clerk's Office of any change to his current mailing address.
