ORDER: Petitioner Eric Dotson has filed a Notice of Appeal [D.E. No. [1O]] but has not paid the $505.00 appellate filing fee or filed a motion to proceed informa pauperis on appeal. Accordingly, the Court ORDERS as follows: 1. The Clerk of the Court shall send Dotson a Sixth Circuit Form 4 and a Certificate of Inmate Account [EDKY Form 523]. 2. Dotson must pay the $505.00 filing fee to the Clerk of the Court or: (a) have prison staff certify the Certificate of Inmate Account; (b) complete the Sixth Circuit Form 4; and (c) file both of these documents with this Court. 3. Dotson must take each of these steps within twenty-eight days or the United States Court of Appeals for the Sixth Circuit will dismiss the appeal for failure to prosecute. 4. The Clerk of the Court shall forward a copy of this Order to the Clerk of the United States Court of Appeals for the Sixth Circuit. Signed by Judge Henry R.
