Clerk shall send Coleman blank forms: a. EDKy 521; b. AO240; and c. EDKy 523. 3. Coleman is welcome to file a new action regarding this same matter. To do so, Coleman must fully complete, sign, and file the EDKy 521 Form. Coleman must also complete and file both the AO 240 and EDKy 523 Forms in order to pursue pauper status, or, alternatively, he must either pay the $5.00 filing fee or at least submit a copy of a completed BP-199 Form that he filed with prison officials requesting that the institution withdraw funds from his inmate account to pay the filing fee in this case.
